1. If, for any reason, you cannot keep a scheduled appointment or will be delayed, please call as soon as possible. Charges may be incurred for appointments canceled less than 24 hours before scheduled appointment time without prior notification.
2. Please allow 30 minutes to 1 hour for all initial first visit appointments and 30-45 minutes for all subsequent appointments.
3. Please download, print, fill out, sign and date these forms with the date of your appointment with the doctor . You will need to have these forms with you at your scheduled appointment date and time, otherwise your appointment will be canceled/rescheduled.
If you are being evaluated for a recent injury or any pain syndrome (neck, back, knee, foot, hip, shoulder), you are welcome to bring with you any recent MRI, CT Scan, or X-Ray reports. We suggest you obtain copies of these reports from the original prescribing Physician or the facility where your scans or x-rays were completed.
